Age Limitation: Candidates age should be between 21 to 32 years as on
25-04-2016.
Educational Details: Candidates should have passed Higher Secondary School
Certificate, +2 examination and passed Health Worker (Female) training course
conducted by the Odisha State Nursing for post 1, passed +2 Science examination
under council of Higher Secondary education for post 2 to 5.
Method Of Selection: Candidates will be selected based on written test.

format along with self attested copies of H.S.C. Examination certificate or
equivalent examination, + 2 Examination certificate or equivalent Examination,
MPHW(F)/ D.Pharm/ DMRT/ DMLT/ GNM Training Pass Certificate, Registration
Certificate of Odisha Nursing council/ DMRT/ DMLT/ Pharmacy council/
Residential Certificate, Caste Certificate, mark sheets of all these documents
by Regd/ Speed post/ courier only & on the top of the envelope superscribed
as “Application for the post of Contractual————– (name of the post)” to the
Chief Dist.Medical Officer, Mayurbhanj, At/PO: Baripada, 75700l on or before
20-05-2016 by 01:00 PM.
